What to expect as you consider joining
This document is called a consent form. The study team will call you to walk you through this. The consent process is designed to help you learn more about RELIANCE. Afterwards, you can decided whether you want to join the study.
You will learn:
What to expect while you participate;
The potential benefits and risks of joining;
The steps RELIANCE is taking to protect your personal information and the data we collect about you;
Who to contact if you have more questions if you decide to join.
